SHIFTS AVAIL 3p-11p MON-FRI
7a-11p SAT &SUN

Responsibilities:

• Administer medications and treatments as prescribed by physicians.
• Monitor patients vital signs and overall health status.
• Assist with tracheostomy and ventilator care, ensuring patient comfort and safety.
• Educate patients and families on care plans and health management.
• Document patient progress and communicate effectively with healthcare teams.
• Provide emotional support and companionship to patients and their families.
• Participate in care planning and interdisciplinary team meetings.
• Maintain a clean and safe working environment in accordance with health regulations.

Requirements:

• Valid LPN license in the state of Pennsylvania.
• Must have attended a Accredited School of Nursing.
• Must be trach and vent certified or readily avail for training.
• Strong communication skills and a compassionate demeanor.
• Ability to work in a Home setting
• Current CPR and First Aid certification.
• Basic computer skills for documentation and communication.
• Flexibility to work various shifts, including evenings and weekends.
• Commitment to providing high-quality patient care.
